[Beta] Adding a risk assessment to a note in the Rula provider portal

Risk assessments should be completed by therapists whenever new, relevant information becomes available or there is a change in the patient’s clinical presentation or circumstances, including but not limited to:

  • Significant life event that had the potential to increase the patient’s risk
  • Significant changes in treatment
  • Patient, family member, collaborating provider, or other entity’s expressed report or concern for the patient’s risk of harm to self or others
  • Increased hostility towards others
  • Additionally, providers may choose to complete a risk assessment at any point in time during care with a patient.

You can add a risk assessment from a session note in the Rula provider portal.

Guided Steps

To add a risk assessment:

  1. Log into the provider portal
  2. Click on the Clients tab, located on the menu on the left side of the page
  3. Select the client you need to add a risk assessment for
  4. Find the appointment for the date you want and click Start progress note or Complete progress note.
    • This wording will reflect the template or appointment type (SOAP note or Initial Assessment).
  5. On the right side of the page, click Add risk assessment

  6. Complete the risk assessment
  7. Click Save & close

The risk assessment will now be updated and tied to the session note.
