Creating and signing a session note in the Rula provider portal

Session notes can be created from a client's chart, located in the Clients tab of the Rula provider portal. Session notes include:

Please note that at this time, the ability to offer appointments in person is only available to California providers. You can read more about in-person offerings here and find which insurances you are credentialed with in the Rula provider portal.

  • Initial Assessment
  • Progress Note
  • SOAP Note

The default note type in the portal for a session that is not an initial assessment is a progress note, however the template can be changed to a SOAP note once the note is created.

Guided Steps

To create a session note for an appointment:

  1. Log into the provider portal
  2. Click on the Clients tab, located on the menu on the left side of the page
  3. Select the client you need to create the note for.
  4. Find the appointment card for the desired date, and click Start progress note

    • This will say Start initial assessment if this is the first appointment with a client. 
    • Once the note opens, you can optionally change the template to a SOAP note, by clicking Change note template on the top left of the note.
    • If you opt-in to see clients in person, you can change your location and address in the Info section of the note. 
  5. Fill out the note, all fields are required.
    • If you need to finish the note later, click Save & close
    • To finish the note, find the appointment card in the client chart and click Complete progress note.

      This wording will reflect the template or appointment type (SOAP note or Initial Assessment).
  6. Once complete, click Sign note.
  7. Confirm your electronic signature, then click Sign and submit note

Once the note is submitted, it cannot be edited. The signed note will generate a charge for the session and be billed to your client and/or their insurance. A separate charge slip is not required for notes in the provider portal. You will see a $ reflected on the note to indicate a charge slip has been completed.

Diagnosis codes will carry over to future session notes and can be edited or adjusted as needed.
