Completing and submitting a discharge note allows you to move clients from “active” to “inactive” to conclude your therapeutic relationship, removing them from your caseload.
Active clients include those who:
- Have a future appointment scheduled (includes pending initial appointments, confirmed initial appointments, and follow-up appointments), OR
- Attended an appointment in the past 90 days, OR
- Canceled an appointment within the past 14 days
Inactive clients include those who:
- Have not scheduled a follow-up visit within 90 days after an initial appointment or within 30 days of the most recent ongoing appointment;
- Scheduled an initial appointment, but the appointment was cancelled, AND
- Provider has completed a discharge note.
