Creating and signing your charge slips

This article includes a video and step-by-step instructions for signing a billable note and charge slip in AdvancedMD. Additionally, it will review the key steps you can take to ensure CPT and diagnosis codes are used correctly.

Complete the steps in this interactive tutorial to practice signing a billable note and charge slip.

Create and Sign your Charge Slip

Please follow the steps below to create and sign your charge slip

  • After signing your note (Create a Session Note), a charge slip should automatically open up with the pre-populated CPT code and diagnosis code.
  • Ensure the CPT and diagnosis codes are correct in the Charge Slip Summary.


  • Click Sign again > enter sign off password 1234 > Sign



Notes and charge slips will not save automatically until they’ve been signed. If you do not finish your note but are not ready to sign it, save your work by clicking Save at the top of the page before closing out the note or charge slip.

CPT and Diagnosis Codes didn't pre-populate

If the CPT and Diagnosis codes didn't pre-populate, you'll have to manually add them to the charge slip. To do this:

  1. Click the Charge Slip tab next to the Appointment tab
  2. Type in the appropriate CPT code in the search bar labeled Procedure Code.

  3. Click on the appropriate code to select it.
  4. In the Diagnosis Code section, type in the diagnosis code or search the lists to find the appropriate diagnosis code.

  5. Click the code to select it.
  6. Both codes should now appear in the Charge Slip Summary on the right.

    Note: If you select the wrong codes, you can clear them from the Charge Slip Summary by clicking on the red circle next to the code. This will remove the codes from the charge slip, and you'll have to then select the appropriate codes.
